ABSTRACT

This systematic review assessed the safety and efficacy of the disposable circumcision suture device (DCSD) and conventional circumcision (CC) in the treatment of redundant prepuce and phimosis. Two independent reviewers conducted a literature search for randomized controlled trials (RCTs) using the DCSD and CC for the treatment of redundant prepuce or phimosis in China and abroad. Nine RCTs (1898 cases) were included. Compared with the CC group, the DCSD group had a shorter operative time (standardized mean difference [SMD] = -21.44; 95% confidence intervals [95% CIs] [-25.08, -17.79]; P < 0.00001), shorter wound healing time (SMD = -3.66; 95% CI [-5.46, -1.85]; P < 0.0001), less intraoperative blood loss (SMD = -9.64; 95% CI [-11.37, -7.90]; P < 0.00001), better cosmetic penile appearance (odds ratio [OR] =8.77; 95% CI [5.90, 13.02]; P < 0.00001), lower intraoperative pain score, lower 24-h postoperative pain score, lower incidence of infection, less incision edema, and fewer adverse events. There were no differences between the CC and DCSD groups in the incidences of dehiscence, or hematoma. The results of this meta-analysis indicate that the DCSD appears to be safer and more effective than CC. However, additional high-quality RCTs with larger study populations are needed.

ABSTRACT

Objective To study the relationship between cognitive impairment and hypercholesterolemia as well as the risk factors thereof in elderly patients with white matter lesions (WML). Methods A total of 347 WML patients were divided into normal cognitive group (n=86) and cognitive impairment group (n=261) according to their mini-mental state examination (MMSE) score. The general situation of the two groups was compared and the correlation of hypercholesterolemia with WML severity and cognitive function was analyzed; the risk factors of cognitive impairment in WML patients were analyzed by logistic regression analysis. Results Compared with those in normal cognitive group, the patients in cognitive impairment group were older, the plasma low-density lipoprotein (LDL) level and the proportion of male, low education level, hypertension and hypercholesterolemia were significantly higher (P<0.05), while MMSE score and the proportion of patients with a history of statins were significantly lower (P<0.05). The proportion of patients with cognitive impairment significantly increased (P=0.001), and the incidence of hypercholesterolemia also increased significantly (P=0.000) with the increasing of WML severity. The Montreal Congnitive Assessment (MoCA) score, visuospatial and executive function, attention and computing power, language and abstract ability, and delayed recall scores of patients with hypercholesterolemia were significantly lower than that of patients without hypercholesterolemia (P<0.05). Logistic regression analysis showed that low education level, hypertension, hypercholesterolemia and the higher level of plasma LDL were independent risk factors of cognitive impairment in patients with WML (P<0.05), while the history of statins use was a protective factor (P<0.05). Conclusion Hypercholesterolemia can significantly increase the risk of cognitive impairment in patients with WML, thus the WML patients with hypercholesterolemia should be focused, and various risk factors should be controlled, at the same time the fortified early intervention is necessary.

ABSTRACT

<p><b>OBJECTIVE</b>To preliminarily study the changes in CD4CD25regulatory T cells (Tregs) in children with severe purulent meningitis at the early stage and its possible implications.</p><p><b>METHODS</b>A retrospective analysis was performed on the clinical data of 39 children with severe purulent meningitis who were admitted to the pediatric intensive care unit from August 2014 to December 2015. According to whether Tregs count was decreased within 12 hours of hospitalization (considering Tregs count <410/mmas decreased), they were divided into two groups: decrease group and non-decrease group. The associations between the changes in Tregs cells and the clinical manifestations, laboratory marker levels, and prognosis were analyzed.</p><p><b>RESULTS</b>Of the 39 cases, 13 (33%) showed a decrease in the proportion of Tregs cells (<31%) and 18 (46%) showed a decrease in the absolute Tregs cell count (<410/mm). Four deaths were all in the Tregs decrease group. Compared with the non-decrease group, the decrease group showed a significantly higher proportion of children with a peripheral blood leukocyte count lower than the normal range and a significantly greater increase in the level of serum procalcitonin (P<0.05).</p><p><b>CONCLUSIONS</b>Tregs might be suppressed in children with severe purulent meningitis at the early stage. And its suppression could be related to the severer inflammation reaction and higher mortality in those patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical effect and safety of circumcision stapler in the treatment of phimosis and redundant prepuce.</p><p><b>METHODS</b>We treated 120 patients with redundant prepuce or phimosis using circumcision stapler and another 60 by conventional dorsal-incision circumcision. We observed intraoperative blood loss, operation time, postoperative pain, wound healing time, cosmetic appearance of the penis, and postoperative complications and compared them between the two groups of patients.</p><p><b>RESULTS</b>Stapler circumcision showed obvious advantages over the conventional method in intraoperative blood loss ([2. 3 ± 1. 3] vs [15.6 ± 2.9] ml), operation time ([7.1 ± 1.4] vs [22.6 ± 4.6] min), wound healing time ([12.0 ± 2.9] as [16.3 ± 3. 1] d), postoperative pain score (1. 9 ± 1. 3 vs 5. 2 ± 1. 7), incision edema, and cosmetic appearance of the penis (all P <0. 05). Besides, stapler circumcision exempted the patients from stitch-removal pain. However, the incidence rate of postoperative local ecchymosis was significantly higher in the circumcision stapler group than in the conventional circumcision group (20. 8% vs 8. 3% , P <0. 05).</p><p><b>CONCLUSION</b>Circumcision stapler, with its advantages of easier manipulation, shorter operation time, better cosmetic penile appearance, less pain, and fewer complications, is superior to conventional circumcision in the treatment of phimosis and redundant prepuce.</p>

ABSTRACT

Glucokinase (GK) is a new target for the treatment of type II diabetes mellitus (T2DM). In order to find a structure-simplified small molecule GK activator, 19 salicylic acid derivatives were designed and synthesized based on new lead compound (1). Experimental results showed that the potency of compound 8h is superior to control RO-28-0450 in GK activation.

ABSTRACT

A novel series of sorafenib analogs containing 2-picolinyl hydrazide moiety were designed and synthesized. In vitro, most of synthesized compounds have antiproliferation activity on MDA-MB-231, ACHN, HepG2, Mia-PaCa-2 and SW1990 cell lines tested by MTT assay. It is worth noting that the antitumor activities of compounds 2c, 2d and 2f are more potent than that of sorafenib on pancreatic cancer cells Mia-PaCa-2 and SW1990, and the activities of compounds 3f and 3g are 2-3 times than that of sorafenib on human hepatocellular carcinoma HepG2 cell line.

ABSTRACT

<p><b>OBJECTIVE</b>To study the radiation-induced blood deficiency and the combination of prescription and syndrome of Siwu Tang using ultra performance liquid chromatography-quadrupoles-time of flight mass spectrometer (UPLC-ESI-Q-TOF-MS), in order to discover the changes in metabolic profiles of blood deficient mice induced by radiation, and clarify the relationship between blood deficient syndrome and the mechanism of Siwu Tang.</p><p><b>METHOD</b>Thirty six C57 mice were randomly divided into three groups: the control group and the model model groups and the Siwu Tang group. The model was established by general irradiation with 3.5 Gy60 Coy ray. The animals were sacrificed on the 7th day after radiation and their blood, spleens and thymus were collected and detected using UPLC-ESI-Q-TOF/MS. MarkerLynx XS software was adopted to identify chromatographic peaks in the total ion chromatogram. Data was processed by making principal component analysis and analyzed by orthogonal partial least squares method among these groups, in order to rapidly identify marks through pattern recognition and analysis.</p><p><b>RESULT</b>Compared with the control group, lysophosphatide, glucosiduronic acid, monoacylglycerol, erythronic acid, ceramide, aspartate phosphate ester, glyceryl phosphatide were obviously changed in the sera of the model group. Monoacylglycerol, ceramide, lysophosphatide, hydroxybutyric acid, palmitinic acid, 3-hydroxystearic acid, diethylarginine, neuraminic acid, phosphatidylserine were found in metabolites of their spleens. Methyladenosine, sitosterol, carnitine, phosphatidylinositol, phosphatidylethanolamine, diglyceride, dimethylarginine, ceramide, hydroxybutyric acid, cholesterol were found in thymus of the model group.</p><p><b>CONCLUSION</b>Analysis on physiological functions of these biological markers show that radiation could induce the disorder of the metabolism of lipoid and carbohydrate and affect the synthesis of some amino acids, and Siwu Tang can reverse these effects.</p>

ABSTRACT

In the present study, an ultra performance liquid chromatography coupled with time-of-fight mass spectrometry (UPLC-TOF/MS) based chemical profiling approach was used to evaluate chemical constitution between co-decoction and mixed decoction of ginseng and Radix Aconiti Praeparata. Two different kinds of decoctions, namely co-decoction of ginseng and Radix Aconiti Praeparata: water extract of mixed two herbs, and mixed decoction of ginseng and Radix Aconiti Praeparata: mixed water extract of each individual herbs, were prepared. Batches of these two kinds of decoction samples were subjected to UPLC-TOF/MS analysis. The datasets of t(R) m/z pairs, ion intensities and sample codes were processed with supervised partial least squared discriminant analysis (OPLS-DA) to holistically compare the difference between these two decoction samples. Significant difference between the two decoction samples was showed in the results of positive ion mode. The contents of hypaconitine and deoxyaconitine decreased, while that of benzoylmesaconine, benzoylhypaconine and dehydrated benzoylmesaconine increased in the samples of co-decoction of ginseng and Radix Aconiti Praeparata. The content of diester-diterpenoid alkaloids decreased, while that of monoester-diterpenoid alkaloids increased, which is probably the basis of toxicity-attenuated action when combined ginseng with Radix Aconiti Praeparata.

ABSTRACT

<p><b>OBJECTIVE</b>To discover the characteristic changes of metabolic profiles in C57 mice with cyclophosphamide induced blood deficiency and the effect of Siwutang.</p><p><b>METHOD</b>An integrated metabonomic study using high-resolution 1H nuclear magnetic resonance (NMR) spectroscopy has been applied to investigate the metabolic profiles of serum, aqueous and lipidic extracts of thoracic gland, spleen, bone marrow obtained from control, model group (intraperitoneal injection of cyclophosphamide at a dose of 250 mg x kg(-1)) and Siwutang treated model group. The NMR spectra were integrated in segments of 0.04 ppm and then analyzed by principal component analysis (PCA) using SIMCA-P software to visualize the similarities and differences in metabolic profiles between these groups.</p><p><b>RESULT</b>PCA result showed conspicuous difference in the metabolic profiles between groups. Compared with the control group, the model group contained lower concentration of lactate, 3-hydroxybutyrate, choline, glucose, and higher concentration of VLDL/LDL, leucine/isoleucine in serum. Lower concentration of taurine choline, Fbeta:RCH2CH2CO, Epi-coprostanol and lactate were found in both in thoracic gland extracts and spleen extracts. And in spleen extracts, we also found the lower concentration of 3-HB. In the extracts of bone marrow, the lower concentration of lactate, choline, glucosee were observed. When they were dosed with Siwutang 10 g x kg(-1) x d(-1) for 7 days, the effects above-mentioned were reversed.</p><p><b>CONCLUSION</b>The injury established by injecting CTX is a kind of proper model to develop further metabonomics researches. The damage of mitochondria, disorder of energy metabolism and osmoregulation are observed in cyclophosphamide caused blood deficiency model by NMR-based-metabonomics method, and the Siwutang can improve these effects.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the essence of blood deficiency syndrome and the mechanism of the effects of siwutang based on the characteristic changes in metabolic profiles of C57 mice with blood deficiency induced by compound method of bleeding, starved feeding and exhausting before and after the treatment of Siwutang.</p><p><b>METHOD</b>Thirty C57 mice were randomly divided into three groups of 10 animals: the control group and two blood deficiency model groups without or with siwutang treatment. The animals were sacrificed after induction by compound method of bleeding,starved feeding and exhausting for 10 days and the blood, spleen, thymus and bone marrow were obtained. The metabolic profiles of serum, aqueous and lipidic extracts of thymus, spleen and bone marrow were investigated using 1H nuclear magnetic resonance spectroscopy (NMR). The NMR spectra were integrated in segments of 0.01, normalized and then analyzed using SIMCA-P software to visualize the similarities and differences in metabolic profiles between these groups.</p><p><b>RESULT</b>Compared with the control group, the model group contained different levels of lactate, choline, glucose, taurine, alanine, LDL, glycerin, UFA, creatinine, acetoacetate, glutamate and beta-hydroxybutyrate (beta-HB). In the Siwutang treated group, the above-mentioned changes were reversed.</p><p><b>CONCLUSION</b>The disorder of energy metabolism, damage of lymphocyte, and disorder of immune function were observed in blood deficiency model by NMR-based-metabonomics method, and the Siwutang can improve these effects. Metabonomics may be a valuable tool in the pharmacological evaluation of siwutang.</p>
